It shows that Mr. Markle believes gay people are inferior in some way, and that by calling Hillary a lesbian it somehow demeans her. I think most people are not one bit concerned about sexual orientation, and I would agree with you if she was having infidelity and there was a trust issue involved in your question, but this was a simple propaganda technique called association which requires most Americans to believe that sexual orientation matters....they simply do not care, and you my friend have given a very obvious "tell".
