Not long ago I had a pinball machine playfield out of the cabinet to work on.
Since the whole bottom of it is crammed with relays and step switches and the rest, there was no way to support it on anything for me to get under it to work on it. So I had the harebrained idea to set each end of it on two tv trays. Only problem was I could only get about a 1/4" of each end on the trays.
Sure enough I got it all set up and started working on it and got damn near the whole job finished. And then all of a sudden it fell off the tv trays and fell to the floor and crushed all the parts underneath rendering it a tore up mess.
I kicked myself in the ass for a week for being so stupid.

I wonder where that happened. Looks like someone failed to inspect their rigging before they used it or that they used rigging that was under the WLL for that lift. Whatever happened, that one goes into the "last day on the job" file. pale

Must have happened at the Port of Gentil in west central Africa.

Video: This is what it looks like to drop a 180-tonne train
yesterday at 7:50 am

As the old saying goes 'the bigger they are, the harder they fall', so
it's no wonder why this 180-tonne train made an almighty noise when it
fell from a crane.

This brand spanking new American Engineered EMD GT46C-ACe work train
was being delivered to a manganese mining company in Gabon, Central West
Africa, when one of the ropes holding train snapped causing it to come
crashing back down to terra firma.

We’re not sure how much an EMD GT46C-ACe would cost you but we have a
feeling that this particular one is a complete write off. You certainly
wouldn’t want to be the guy who tied those ropes together in the first
place.

My guess is that they used steel cable that was rated for the load, within a margin of safety and then did something stupid like not shackle it up properly (or wrapped bare cable around a sharp edge).

Occasionally, I make the 4-hour drive between Pensacola and Birmingham with a trailer load of stuff and I put extra lines on things to make sure NOTHING moves inside. If I use a canvas tarp, there WILL NOT be any flapping tarp, I can assure you. I'll spend a half hour sometimes running lines every which-a-way. (It's amazing how quickly all those lines can be untied, compared to the time it takes to tie 'em.)

Looking at the video they maybe have been using strap instead of wire rope. It may have cut it on the sharp edge of the bottom of the load. I have rigged some very heavy lifts, it always makes you breath shallow until the load is set. Looking at it again They may have had a strap running under the load then they tied it to cable/straps on the spreader bar. Eric may be close. Looks like it failed where the 2 straps joined together. Just glad it wasnt me. .
